How to get a GPCCMP referral

Simply visit your GP and ask for a GP Chronic Condition Management Plan (GPCCMP), the Medicare item that replaced the former Enhanced Primary Care (EPC) / Chronic Disease Management (CDM) plan. Your GP will assess your eligibility and, if appropriate, refer you to Angus or Anastasia as your allied health provider.

You are eligible for up to 5 allied health visits per calendar year across all allied health providers combined (e.g. physiotherapist, podiatrist, dietitian). Unused visits do not carry over to the next year.

Option 3

Common Questions

Frequently Asked Questions

Do I need a referral to see a dietitian?

No referral is needed for a private consultation — you can book directly with Angus or Anastasia at any time. A GP referral is only required if you wish to access Medicare rebates through the GP Chronic Condition Management Plan (GPCCMP).

Visit your regular GP and ask for a GP Chronic Condition Management Plan (GPCCMP) — the Medicare item that replaced the former Enhanced Primary Care (EPC) / Chronic Disease Management (CDM) plan. Your GP will assess whether you are eligible. If approved, they will refer you to Angus or Anastasia as an allied health provider and you can then contact us to make an appointment.

Medicare covers up to 5 allied health visits per calendar year (1 January to 31 December) across all allied health providers combined — this includes dietitians, physiotherapists, podiatrists, and others. Unused visits do not roll over to the following year.

Yes. If you hold a valid concession card — such as a Health Care Card or Pensioner Concession Card — and have a GPCCMP referral from your GP, your review consultations will be bulk billed with no out-of-pocket cost. A reduced gap fee applies to initial concession consultations.

Many private health insurers include dietitian services under their extras (ancillary) cover. The rebate amount varies depending on your policy and fund. We recommend contacting your insurer directly before your appointment to confirm your entitlements. You will receive a receipt after your consultation to submit a claim.

If you are self-managed, you pay for your appointment and then submit a claim to the NDIS for reimbursement. If you are plan-managed, your plan manager pays Effective Dietetics directly from your NDIS funds. Please contact us before your first appointment so we can set up the correct invoicing process for your situation.

Yes. Both Angus and Anastasia are available for private, GPCCMP, NDIS, DVA and RACF appointments. Let us know your preference when booking, or we can help match you with the right practitioner for your needs.

Payment is accepted on the day of your appointment. We accept cash, EFTPOS/card, and bank transfer. Please contact the clinic directly to confirm the payment options available at your preferred location.

We ask that you give us as much notice as possible if you need to reschedule or cancel your appointment. Please contact us at least 24 hours in advance wherever possible so that we can offer your appointment time to another patient.
